The CMPFE program, administered by the Department of Elder Affairs and operated in all 99 Iowa counties by the Area Agencies on Aging, is the network that coordinates home and community based services for frail and vulnerable Iowans aged 60 and older. The program also serves Iowans qualifying for the Title XIX Home and Community based services (Medicaid) Elderly Waiver. For more information on this waiver, read the description under the Elderly Waiver Program. Direct services provided by Iowa's Senior Living Trust and the Administration on Aging National Family Caregiver Program are also coordinated through the CMPFE program.

CMPFE serves Iowans aged 60 and over who have one or more health care needs. At the end of Fiscal Year 2007, there were 8,979 active consumers participating in CMPFE with 13,291 total consumers served.

The program is designed to prevent premature institutionalization of the elderly by arranging for appropriate services. To enroll in CMPFE, contact your local Area Agency on Aging. The case manager, the client, and anyone else the client would like, develop an individualized plan of care. Some services available through CMPFE include:

• Home delivered meals
• Emergency response system
• Nursing and homemaker services
• Chore Services
• Assistive Devices
• Adult day services and respite
• Transportation

The process begins when the case manager makes an appointment with the client to come to his or her home to conduct an assessment of the client's needs. It is the client's choice if he or she would like a family member or friend to be present while the assessment is being completed. After the assessment is completed, the case manager will talk with the client about what services might work best for the client based on the client's circumstances. The client is given information about the agencies that provide those services in the community. Together the case manager and client develop a personalized plan of care.

The client makes the final decisions on the providers and services that he or she will receive. There is no cost to the client for Case Management, but the client may have to pay for the services provided. Some services are offered on a sliding fee scale.

After the client makes the final decision about the services, the case manager will arrange for the services and will check on the delivery of services to make sure they meet the client's needs. The case manager will continue to be available to coordinate the plan and act as the client's advocate. At least once a year, another assessment will be done. At that time, the client's plan of care will be reviewed to see if changes need to be made.

Elderly Waiver Program

Another program that is available is the Medicaid Home and Community Based Services-Elderly Waiver Program. In order to receive in-home services under this program, clients must be 65 years of age or older, meet income and resource requirements, and have significant health needs. Services received through the Elderly Waiver program are paid for by Medicaid dollars. There is a monthly maximum of $1,084 per individual if the client meets the intermediate level of care and $2,554. if the client meets the skilled level of care.
