This report is filed by certain officials and state employees to disclose primary sources of income and other financial holdings for the previous calendar year. The report must be filed by April 30 of each year. Candidates for statewide office file the report by April 30 the year in which such candidates are on the ballot. Effective December 23, 2009, these reports are required to be filed electronically.
