Amounts of income, specific addresses, and account numbers are not required in this statement. Only the source and type of income, name of business and type of business as well as the position held are required. Please see Iowa Code 68B.35 and Board rules in Chapter 7.
In each section of the report, you must click the “add” button to add your selection to the report. If you have more than one position in which you are required to file, you may report both on one report by choosing an additional agency, department/division, and position, then clicking “add” once more. If you do not see your agency, department/division, or your position listed, use the box marked “other” as appropriate. If you are a candidate for statewide office, please enter the office you will be running for.
You must enter your name in Last Name, First Name format. If you have filed in previous years, the system will populate the rest of the fields for you. Just select and make any changes necessary. The year filed is always the year PRIOR to the year you file. For example, statements filed April 2012 are for the 2011 covered year.
You are required to select an agency, then a department or division, then position to file the report. Click the "add" button after selecting. If an agency does NOT have a department or division, choose the none/not applicable selection to trigger the position field to be populated. If the agency, department, or position does not exist, you may use the “other” field to add it to the listing, then click or select “add” to attach it to your report. If you are filing as a candidate for a statewide position, please choose statewide, then candidate, then the position. If you do not click “add” the next button will not become active.
Income Source Help
Enter each source from which you received more than $1,000 in gross annual income during the previous year. The amount or value of the holding is not required to be listed. Income includes the total amount of any income received jointly with one or more persons exceeding $1,000. Do not report income received solely by your spouse or other family members. A source is reportable if the gross income produced was subject to federal or state income tax during the reporting period. You must click “add” after each selection to add this to your report. If you do not have income in a section, click the appropriate checkbox to the bottom of that section to proceed.
Upon completion of the report, you will be presented with a review. Please check carefully and make necessary changes by using the previous button. Once complete and correct, check the affirmation box and submit.
