| This report is filed by an agency when it
receives a gift or bequest. The Governor also files
the report when receiving a gift on behalf of the state.
Internal tracking systems that accurately
capture appropriate information may also be used in lieu
of filing reports. By agreement among the Government Oversight Committees and the Board, reports filed with the Board are deemed to be sufficient to meet the statutory requirement that these reports also be filed with the Oversight Committees. See
Iowa Code section 8.7. |

| This report
is filed by certain officials, state employees, and candidates for statewide office for the
preceding calendar year to disclose primary sources of
income and other financial holdings. The report must be
filed no later than April 30 of the year following. Candidates
for statewide office file the report on April 30 of the year
in which the candidates are on the ballot. See Iowa Code
sections 68B.35 and 68B.3(2), and rules in 351 – Chapter
7. |

Governor's Office Consent for Sales/Leases |
| The Governor's office creates its own form
to file with the Board. This report is filed when a full-time
member of the Governor's Office obtains consent to sell
or lease a good or service to a registered lobbyist or
to an individual, association, or corporation that employs
a registered lobbyist. The report must be filed with the
Board within 20 days of consent being granted. See Iowa
Code section 68B.4B and rule 351
– 6.12. |

Regulatory Agency Consent for Sales/Leases |
| Each regulatory agency creates its own consent
form to file with the Board. This report is to be filed
when an official or employee receives consent to sell or
lease a good or service to individuals, associations, or
corporations subject to the official's or employee's regulatory
agency. The report must be filed with the Board within
20 days of consent being granted. See Iowa Code Section
68B.4 and rule 351
– 6.11. |

| This report must be filed by the sponsor
of a function held during the regular legislative session
when all 150 legislators are invited. All state and local
government personnel may attend the function and the $3.00
gift law does not apply. The report must be filed with
the legislative branch within 5 business days of the function
and the legislative branch then makes copies available
to the Board. See Iowa Code section 68B.22(4)"s." |

| Pursuant to Iowa Code section 68B.2B (see
2006 Iowa Acts, HF 2593, sec. 1), an official or employee
accepting simultaneous employment with another executive
branch agency shall file notice of the dual employment
within 20 days of accepting the second employment. |
