The Iowa Department of Inspections and Appeals (DIA) welcomes comments and questions from its customers and stakeholders. All communications with the Department, whether verbal, written or electronically submitted, will be acknowledged and, if warranted, the appropriate action will be initiated.
Complaints about any entities regulated by DIA, such as nursing facilities, restaurants, etc., may be submitted in any form and need not be signed. DIA will investigate all valid complaints and respond to the complainant if a return address is provided. Note: All complaints will be treated as confidential correspondence.
