Please include your license number, previous name/home address, and current name/ home address. Employment address is not acceptable. IF YOU ARE AN LPN IN IOWA AND HAVE APPLIED FOR THE RN EXAMINATION, YOU WILL NEED TO DIRECT YOUR CHANGE OF ADDRESS TO THE NEW LICENSURE DEPARTMENT: Email Change of Address, SO THAT BOTH RECORDS ARE UPDATED. WHEN SUBMITTING CHANGES, PLEASE INCLUDE YOUR LPN LICENSE NUMBER OR SOCIAL SECURITY NUMBER FOR IDENTIFICATION PURPOSES. Address changes can also be updated on our web-site. Click on the link below: Name changes are not available online. Licensees who notify the board of a name change will not automatically receive a license card in the new name. If a license in the new name is desired, there is a fee of $20.00 for a re-issued/duplicate license. If you would like a re-issued wallet card, send a written request and the current wallet card including the $20.00 fee. If you do not have in your possession a current wallet card you will need to complete the request for duplicate wallet card form, Application for Duplicate Wallet Card or Certificate which will need to be notarized and mailed to this office along with the $20.00 fee.
