 along with an IA 1040-V (pdf) Payment Voucher payable to:

Treasurer, State of Iowa. Payments must be at least $1. Write the type of tax being paid and the tax year being paid on the check or money order. The address for mailing is on the IA1040-V Payment Voucher.
