Enter your voluntary contributions to any of the following checkoffs in
boxes 59a, 59b, 59c, and 59d. Please note that you may contribute to any
of the checkoffs regardless of whether you are entitled to a refund or owe
additional
taxes, but your contribution will reduce your refund or add to the amount
you owe. Your contribution will qualify as a charitable contribution on your
2006 return if the return
is filed during the 2006 calendar year. If you file an amended return, you
cannot change your contribution.

FISH AND WILDLIFE FUND
(Chickadee Checkoff). You
may contribute any amount of $1 or more to the Iowa Fish and Wildlife
Protection Fund. Your contribution through this checkoff is the primary
support for Iowas Wildlife Diversity Program which monitors,
researches and manages the states nongame species of wildlife.
According to the Natural Resource Commission policy, 100% of the money
donated to this program goes to the Wildlife Diversity Program.

CORN DOG CHECKOFF.
You may contribute
any amount of $1 or more to this fund. The proceeds from this checkoff
will be added to other sources such as gifts, donations and bequests
to be used by the Iowa State Fair Blue Ribbon Foundation to
fund capital projects and improvements to property on the Iowa State
Fairgrounds.

KEEP IOWA BEAUTIFUL CHECKOFF.
You may contribute
any amount of $1 or more to this fund. The proceeds from the checkoff
will be used to educate and encourage Iowans in ways to enhance the
beauty of this state.

FIREFIGHTERS CHECKOFF.
You may contribute
any amount of $1 or more to this fund. This checkoff will
be used to train Iowa's firefighters. The money collected will be used
to offset the financial burdens that many fire departments deal with
when it comes to ensuring their firefighters are adequately trained
in all facets of emergency response.
